Description: Clinic Care Coordinator (case management) positions within the Surgery Outpatient Clinic to collaborate with nursing staff and providers for patient care. Requirement: Three (3) year Case Management Experience. Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays.
