Job Details
Job Summary (List Format): - Provide skilled nursing care and case management to patients in their homes or at the in-patient Hospice Center. - Admit patients onto hospice/home health services. - Coordinate and integrate services to ensure continuity of care. - Facilitate achievement of quality clinical and cost-effective outcomes. - Negotiate, procure, and coordinate necessary services and resources for patients and families. - Use the nursing process (assessment, planning, implementation, evaluation) for patient care, including family education. - Work collaboratively with an interdisciplinary team. - Observe, assess, and plan interventions for patients' physical and psychosocial needs. - Oversee and evaluate care provided by Home Health/Hospice Aides and CNAs. - Ensure continuity of care through effective communication and case conferences with other care providers and agencies. - Provide quality care and symptom management for optimal patient health and quality of life. - Maintain accurate and complete patient records per agency standards and OASIS requirements. - Participate in team scheduling, including on-call rotations and after-hours patient assessments as needed. - Engage in quality improvement activities and attend required trainings. - Perform other duties as assigned by supervisor. Minimum Qualifications: - Current RN license in Montana (BSN preferred). - Valid driver's license, auto insurance, and reliable transportation. - 2 years of recent acute care, home care, or hospice experience preferred (including venipuncture, catheterization, wound care, or parenteral therapy). Schedule: - In-home RN: Mon-Fri 8am-5pm. - In-patient RN: Three rotating 12-hour shifts. - Schedule may vary based on patient needs.
